At EVI, personalized care means having the time and space to truly know our patients. We intentionally care for a limited number of Personalized Care members so we can remain present, responsive, and deeply invested in each person we serve.
For the remainder of 2026, we will welcome up to 50 Personalized Care patients. Once those spaces are filled, new patients interested in ongoing care will be invited to join our waitlist.

EVI Personalized Care is designed for women who want an ongoing, relationship-based approach to their health.
Your membership may include:
30-minute follow-up visits as clinically indicated
Ongoing hormone, metabolic, weight and whole-person wellness management
Medication and prescription management
Lab monitoring and interpretation related to your treatment plan
Treatment adjustments when clinically appropriate
Secure EHR messaging for appropriate questions between visits
Minor acute-care support when appropriate
Quarterly wellness check-ins
Personalized education and wellness resources
Priority scheduling when concerns arise
Full EVI Community membership, including EVI Flow, education and select wellness experiences
Your care is personalized to you, so the frequency and type of medical care you receive will depend on your individual needs.
Labs, medications, supplements, imaging and outside services are not included in the membership price.

Many women come to EVI because they're looking for something different from the conventional healthcare experience.
They want time to ask questions. They want to freely discuss what's going on without feeling rushed. They want a provider who knows their story, explains the why behind recommendations, educates them about their health, and looks at the bigger picture—not just an isolated symptom.
I believe good healthcare is built through relationship and partnership.
That's why I created the EVI membership model.
Your care shouldn't be limited to the few minutes we're sitting together in an exam room. Sometimes we need to review new results, adjust a medication, titrate a dose, answer a question, or see each other sooner than expected.
Membership gives us the flexibility to do that without making you wonder whether every additional conversation or clinically appropriate follow-up means another visit charge.
If you're looking for something different from the conventional healthcare experience, this may be your opportunity to invest in you.

No. Labs, medications and supplements are not included in the base membership price.
Your care including labs is personalized to you. I don't believe every patient needs the exact same panel of labs simply because she walks through the door. Once I understand your history, symptoms, goals and concerns, I'll recommend testing based on you.
The same is true for medications. Prescriptions are recommended only when clinically appropriate and individualized to your needs.
EVI Personalized Care members receive preferred/discounted pricing on certain lab services when available. Your specific costs will depend on the testing recommended.

It depends on the testing we decide is appropriate for you.
For maximum convenience, many labs can be drawn right here at our Tollgate office. Certain specialized tests may need to be completed through another laboratory or testing service.
We'll walk you through exactly where to go and what to expect when your labs are ordered.

EVI Medical is located inside Longevity Chiropractic in Tollgate.
When you arrive, simply take a seat in the waiting room. I'll come greet you when it's time for your appointment and bring you back for your visit.

Your first visit is intentionally different.
We'll spend 75 minutes together getting to know you—not just your symptoms, but your health history, lifestyle, concerns, experiences and goals.
I don't take this part lightly because all of those pieces contribute to what makes you, you.
We'll discuss any testing I recommend based on what we uncover together. Once your testing is completed, you'll return for a dedicated 45-minute results and wellness-plan visit.
That second visit is focused on helping you understand your results, discussing my recommendations and developing your personalized plan. Depending on your needs, that may include lifestyle, nutrition, movement, supplements, medications or other recommendations.
If ongoing medical care is desired and appropriate after your assessment, you may then transition into EVI Personalized Care.
Membership is optional following your assessment; however, an active Personalized Care membership is required if you would like to continue receiving ongoing medical services through EVI.

Your Comprehensive Wellness Assessment can stand on its own. You'll receive your assessment, recommended testing, dedicated results review and personalized plan without an obligation to continue.
If you decide you'd like EVI to provide your ongoing medical management, you'll transition into EVI Personalized Care.
To begin Personalized Care, your EVI Comprehensive Wellness Assessment must have been completed within the previous 90 days. If more time has passed, we may need to complete an updated assessment before establishing ongoing care.

EVI Community does not require a medical relationship with EVI.
It's designed for women who want to learn, move, connect and grow together.
Community membership includes EVI wellness events, EVI Flow movement experiences right here in Tollgate, educational resources and an online community filled with encouragement, conversation and wellness education.
You can be part of EVI without becoming an EVI Medical patient.

Yes. Telehealth appointments are available for eligible patients located in Tennessee at the time of their appointment, when telehealth is clinically appropriate.
Some evaluations, testing or concerns may still require an in-person visit.

EVI is designed to provide personalized women's wellness and ongoing medical care within the services and scope offered by the practice.
We encourage patients to maintain appropriate relationships with their primary care provider and other specialists. Some health concerns, screenings, emergencies and conditions are best managed by—or in coordination with—those providers.
EVI is about adding time, partnership, education and personalized support to your healthcare—not trying to be everything to everyone.

EVI Personalized Care provides access for appropriate questions and select minor concerns, but EVI is not an emergency or 24-hour urgent-care service.
If you are experiencing a medical emergency, call 911 or seek emergency medical care. Certain acute concerns may also be better evaluated through your primary care provider or an urgent-care facility.
